A. Fellowship Program
The objective of this one-year upper limb orthopaedic fellowship is to train a qualified orthopaedic surgeon to subspecialty-level knowledge and skills in the diagnosis and management of trauma and elective upper limb conditions to function independently as an upper limb specialist.
This will be achieved through exposure to all aspects of upper limb orthopaedic surgery, both clinical and surgical, and will encompass the management of both elective and trauma cases. Elective cases will include exposure to a wide variety of surgical procedures including, but not limited to, hand and shoulder arthroplasty, soft tissue tendon transfers, brachial plexus surgery, post-trauma reconstructions, congenital conditions, management of tumours of the hand, conditions related to inflammatory arthritis, wrist and shoulder arthroscopy and open management of sports-related shoulder, elbow and wrist injuries. In addition, the fellowship is hosted in conjunction with the Tygerberg orthopaedic trauma service, allowing for experience in trauma and fracture management related to the upper limb.
The caseload, wherein the trainee is either the primary surgeon or first assistant, will be of sufficient number to gain proficiency in the management of these cases at an upper limb sub-speciality level. The upper limb fellowship program promotes clinical excellence in managing complex upper limb surgery under the guidance of experienced consultants.
B. Participating Sites
The fellowship will be hosted at Tygerberg Hospital and Groote Schuur Hospital, with the approval of the Western Cape Department of Health. The University of Stellenbosch Faculty of Medicine and Health Sciences and the endorsement of the South African Orthopaedic Association,

Upon completion, the Fellowship will be endorsed by the South African Orthopaedic Association (SAOA) if all the endorsement criteria have been met.

The Fellow will attend and participate in the academic activities hosted by the Orthopaedic department. These include:
- Trauma handover meeting
- General Academic program (weekly)
- Registrar teaching sessions
- Pre- and post-operative planning sessions
- Fellow specific teaching sessions
- Other appropriate courses and congresses held in South Africa
E. Fellow Appointments (Eligibility of Fellow)
- Appointments coordinated by the Director of the Fellowship and the Head of Department. (Suitable candidate's CVs will be assessed, and interviews conducted)
- Contract / Agreement - signed by the Fellow and the Director / Head of Department
- Registration as an affiliated post-graduate student at the University of Stellenbosch
- Registration at the Health Professions Council of South Africa (HPCSA) – Category: Medical Practitioner Post-Graduate studies

F. Clinical Responsibilities (See Addendum A for more detail)
- Daily trauma handover meeting
- Monday: Elective shoulder clinic / Elective lists
- Tuesday: Elective Lists
- Wednesday: Elective hand clinic / Upper limb extra trauma theatre list
- Thursday: Elective hands theatre or trauma list
- Friday: Shoulder elective theatre list.
- Duty hours in accordance with the Department of Health (40 Hours / Week)
- Extensive Ward rounds throughout the week
- Intermittent Outreach visits to Metro East district hospitals
G. Remuneration
- Industry-sponsored stipend @ ZAR 65 000.00 per month (x 12 months), self-funded applicants are welcome to apply.
H. General
- Elective supervision and Trauma supervision: Fellows are allocated to the Upper Limb unit under the direct supervision of the Upper Limb Consultants
- Professional Insurance - covered by The Department of Health as per the agreement, but additional personal medical indemnity also required
- The duration of the Fellowship will be one year (6 months only to be considered in exceptional circumstances)
- Leave: Annual leave of 21 days per year and up to 10 days of study leave per year
- There are no on-call commitments
- No independent private practise activities allowed during the workweek
I. Quarterly Assessments
- Fellows clinical skills and progress will be assessed by the upper limb consultants using the 360 assessment Tools
- The Fellow will be allowed to provide feedback on the fellowship
- Fellows to keep a surgical Logbook - this will be assessed to ensure that an adequate amount of procedures and an acceptable variety of surgical procedures has been provided
J. Research
- Research relating to the field of Upper Limb Surgery will be identified, and the fellow will be required to be involved in research during the fellowship
- Funding can be applied for through The Orthopaedic Research fund
- The fellow will obtain ethics approval through the Bio-ethics committee of Stellenbosch University
- Literature search service, textbooks and journals available through the University of Stellenbosch
